  • The memberships at GP Health are designed to work without insurance. If you have medical insurance, we encourage you to maintain the coverage in case of an emergency. We do not bill your insurance company for care received at GP Health, which allows us to keep our administrative costs low.

  • Yes, you can! You may use your Health Savings Account or Flexible Spending Account to cover the cost of your membership in GP Health and purchase supplements or other items from our store.

  • Because GP Health is an independent platform that does not bill insurance, we are not considered a participating practice, and therefore do not meet the requirements to be listed as your PCP. Our mission is to take a comprehensive approach to your health, so we will gladly continue and expand on the conversation you’re having with your PCP in regards to any health concerns.

  • Since GP Health is not contracted with Medicare, we are unable to accept patients with Medicare coverage due to their compliance requirements. We are working towards a solution where we will be able to accept Medicare clients in the future.

  • Each membership includes a comprehensive lifestyle assessment by your provider. That is then followed by a series of progress visits to work towards the goals identified in your initial assessment. Follow-up visits are conducted virtually. All members have access to our HIPAA-compliant secure member portal and curated digital health tools throughout the year.

    Membership rates are listed below. A 10% discount is applied if the year is paid upfront.

    Sage: 1 lifestyle assessment, 3 progress visits (every 3 months), $175/month

    Berry: 1 lifestyle assessment, 2 progress visits (every 4 months), $150/month

    Sunflower: 1 lifestyle assessment, 1 progress visit (every 6 months), $125/month

  • The initial exam is an in-person visit. Pre-Visit questionnaires will be provided to you via our secure member portal before the first appointment. During the Comprehensive Lifestyle Exam, we will assess your overall health, current health maintenance needs and decide on appropriate screening or preventative labs if needed. Formal lifestyle and health recommendations will be made during this visit. In addition, we will set your health and wellness goals for the year. The course of care delivered via the HIPAA-compliant telehealth platform will be based on member engagement; thus, it will be expected that members act on the information given and we will follow the progress at subsequent visits. Our conversation may include the following topics:

    • Movement, exercise, and physical activity

    • Improved nutrition and dietary intake

    • Behavioral modification for prevention and management of chronic conditions

    • Sleep and stress reduction

    • Recommended health maintenance, diagnostic test, and interpretation of the same.

    • Mental health and well-being

    • Mind-Spirit-Lifestyle modification and balancing

    • Various therapeutic interventions

  • After your custom treatment plan is established, we will follow up via 30-minute telehealth visits to review, assess, update, motivate, and support progress towards goals set during the annual comprehensive lifestyle exam. The number and cadence of visits will depend on your membership level and desired level of engagement.

  • We will provide you with educational content in addition to year-long access to digital resources that are specific to your health challenges and wellness goals.

  • Sure! We will provide access and support to various digital health tools to shepherd you on your journey to better health. You can access these modules via your smartphone or other web-connected device.

  • The amount of scheduled follow-ups depends on your desired membership level, so the choice is yours!

  • We encourage you to assess where your health is now and where you would like it to be.

    Some members may have chronic conditions or other challenges that require more care to reach their desired goals. Others may wish to engage more to optimize their health. In that case, a membership plan with more progress visits will provide more frequent support. If you are comfortable with your current health status and need a periodic check in from time to time, a membership with fewer progress visits will work nicely.

    You can switch membership plans as desired within the first 3 months of your membership or (if needed) we may suggest a plan with more engagement to help you achieve your goals for the year! You are in control, we are here to help!

  • All scheduling is conducted online via our secure member portal. Once you have activated your membership you will receive a link to the portal. The portal allows you to schedule anywhere you have internet access!

  • We strongly encourage all communication takes place via our secure member portal. Your privacy is important to us and we want to make sure your information is protected. General inquiries can be directed to our email.

  • We will be happy to submit refill requests for medications our practice prescribed. Other medications should be refilled by the physician who prescribed them to you. If you have any questions, please reach out via the secure member portal and we will respond promptly.

  • We encourage you to maintain your relationship with your PCP. They can be especially helpful if you need to connect with specialists.

  • Most visits are conducted virtually via your smartphone or personal device. In-person assessments are by appointment only, so please schedule your appointment via the member portal online and you will receive a confirmation with time, date, and address.

  • Anyone aged 18-64 is welcome to become a member!

  • No. If you have a medical emergency please call 911 immediately. You may also visit your nearest urgent care center or emergency room.
